SEVENTH ANNUAL

NICK ERDY FOUNDATION

GOLF OUTING

JUNE 30, 2012

-Registration opens at 11:00am and the scramble begins between 12:00 and 12:30pm.

-The cost is $80 per golfer, which is tax deductible.

-Proceeds of the event benefit several local scholarships as
well as the Injured Marines Fund.

-The event includes 18 holes of golf with a cart, driving
range, door prizes, 2 drink tickets and a dinner with awards.

-We will present awards to 1st, 2nd and 3rd place as well as the winner of the Longest Drive, Closest to Pin and Longest Putt.

-If you are not a golfer, you can attend the dinner for $12.

-You can also sponsor a hole (see the attached form for details).

-Dinner starts between 5:00 and 5:30pm and will be held at Deer Track Golf Course.

-Please complete the registration form(s) and return by June
11th.
